Entry Requirements
Astaroth resides in the fourth tier of Escalating Nightmares, a series of dungeons that increase in intensity with each stage. To face him, you'll need an Escalation Sigil, which is acquired through the Seasonal Reputation reward system or obtained from the Grand Horadric Cache within Horadric Strongrooms.
Affix Conditions and Preparation
Before facing Astaroth and his flaming beast, be aware of the additional hazards: both general Nightmare modifiers and a special unique affix tied to this specific battle. These modifiers are assigned upon completing the third dungeon in the escalation sequence.
An example setup might include Elite Summons (unique to Astaroth) in combination with general affixes like Volcanic or Poisonous Enemies. These effects persist into the boss arena, creating extra chaos beyond the boss’s own mechanics.
Phase 1: Dual Threat
In the initial stage, Astaroth rides the Amalgam of Rage. They alternate actions but never strike simultaneously.

Avoid staying still and keep an eye on meteor impact zones. Survive long enough and once the Amalgam falls, you'll transition to phase two.
Phase 2: Rising Fury
The Amalgam collapses in the arena’s center, slowly regenerating. Astaroth dismounts and becomes more aggressive.
Blood Pool: A circle of blood forms beneath the Amalgam, expanding steadily.
Close-Combat Swings: Astaroth lashes out with melee strikes.
Bounded Rain of Chaos: Meteors hit arena edges, leaving pockets of safety.
Fire Ring Waves: Circles of flame swirl along the perimeter.
Infernal Charge: A fire rush accompanied by linear blasts.
Tactics
Use remaining space wisely, and stay in the shrinking safe zones. Bringing down Astaroth before the Amalgam heals ends the encounter. Otherwise, prepare for phase three.
Phase 3: Overwhelming Assault
With the Amalgam revived, both foes attack at once. Their powers now combine.
Basic Attacks: Astaroth and the hound close in aggressively.
Pack Summon: A group of wolves joins the brawl.
Wide Fire Breath: The Amalgam leaps and casts a broad arc of fire beams.
Teleport Assault: Astaroth warps and immediately launches a skill.
Repeat of Slash and Burn, Charge, Chaos Rain: All upgraded with faster execution.
Fire Storm: Ringed meteor drops overlap with the Amalgam’s wide flames.
Tactics
Focus fire on Astaroth. Killing him ends the fight, while downing the beast reverts to phase two. Watch its path closely, avoid its jaws, and never lose track of Astaroth’s positioning amid the visual chaos.

Consider crafting a Fire Resistance II Elixir to ease the pain of incoming fire damage. It may make the difference in the final phase.
