Introduction

Welcome to this detailed Theater of Pain dungeon guide for season 2 of The War Within! Here, we’ll review the abilities of all trash mobs and bosses in the dungeon, along with strategies to handle them effectively. While focusing mainly on Mythic and Mythic+ versions, much of this guide applies to normal and heroic difficulties as well. Included is a suggested route optimized for PUG groups via the Mythic Dungeon Tools addon.

An Affront of Challengers Trash

Unyielding Contender
Use soothe effects to remove the Death Wish enrage when applied, otherwise tanks should prepare defensives. They charge the tank if moved too far away, so be cautious when repositioning.

Battlefield Ritualist
Interrupt the random Unholy Fervor cast or use Magic dispels to remove the debuff. Spare interrupts should be used on Necrotic Bolt, which targets random players.

Raging Bloodhorn
This mob cannot be crowd controlled. Its Raging Tantrum enrages it, causing heavy group damage for 4 seconds. Use soothe if available to remove it or rely on defensive and healing cooldowns.


An Affront of Challengers Boss Fight

This fight involves three bosses simultaneously, requiring you to cleave them down evenly to avoid stacking Final Will buffs on surviving bosses. Avoid the arena edges where combatants knock players back on proximity.

Sathel requires interrupting Necromantic Bolt to ease tank damage. He applies Withering Touch, a magic debuff on two players—heal teams should dispel these quickly. At full energy, Sathel applies Searing Death, dealing shadow damage that splashes to nearby players, so spread out loosely and use defensives when taking extra damage.

Paceran’s Decaying Breath frontal attack is lethal and random; avoid it carefully. His full energy move, Noxious Spores, spawns puddles under every player repeatedly, so cluster these in one area and rotate the group to manage space efficiently.

Dessia tanks take Mortal Strike, which also reduces healing for 5 seconds; use defensives here. At full energy, she casts Mighty Smash, hitting all players with nature damage and slowing them for 10 seconds—again, use defensives if taking other damage simultaneously.

Xav the Unfallen Trash

Shambling Arbalest
Jagged Quarrel targets a random player—use defensives or bleed cleanses if available. Also watch out for additional random target damage from Shoot.

Ancient Captain
This mob cannot be controlled by crowd control effects. It buffs nearby allies, reducing AoE damage they take, so prioritize killing it first. Interrupt every Demoralizing Shout cast. Shield Bash targets the tank but isn’t too dangerous.

In this area, you’ll encounter three sets of dueling mini bosses. Only one from each pair will appear per run, with the winners randomly chosen as you approach. These duels are Dokigg vs Nekthara, Heavin vs Harugia, and Rek vs Advent. No two bosses from the same duel appear together.

Dokigg the Brutalizer
Immune to crowd control. Avoid the Whirlwind AoE around him. Earthcrusher hits the whole party and leaves damaging circles—avoid these. Tanks should use defensives for the Savage Flurry channel.

Nekthara the Mangler
Also immune to CC. Avoid Whirlwind AoE. Stop casting when Interrupting Roar occurs. Whirling Blade targets 3 random players—avoid being hit.

Heavin the Breaker
Immune to CC. Stop casting during Interrupting Roar. Dodge random target Ground Smash frontal. Tanks should use defensives for Colossus Smash and its damage amp.

Harugia the Bloodthirsty
Immune to CC. Avoid Ricocheting Blade aimed at random players; use defensives or line of sight. Stay out of the Bloodthirsty Charge circles.

Rek the Hardened
Immune to CC. Avoid Whirlwind AoE. When Swift Strikes is active, 2 random players take extra melee damage—use defensives and healing as needed. Tanks beware of Unbalancing Blow and use defensives.

Advent Nevermore
Immune to CC. Avoid Ricocheting Blade attacks and use defensives or LOS. Seismic Stomp hits frequently—healers must keep everyone topped off and use defensives.


Xav the Unfallen Boss Fight

Xav periodically spawns Oppressive Banners that stack a 15% slow on all players. Drag the boss to these and cleave them down quickly. When the boss hits full energy, he uses Might of Maldraxxus, jumping to the arena’s center and hitting all players, followed by a random set of three abilities: Deafening Crash (12-yard AoE interrupt), Massive Cleave (frontal cleaving half the arena), and Crushing Slam (random line attack)—be ready to dodge these.

At 100 energy, Blood and Glory triggers, throwing two random DPS into the pit for a duel. The winner gains a buff, the loser a debuff. Ending the duel quickly is key; players with cooldowns should time them optimally.

Tanks should prepare defensives for the Brutal Combo channel.

Kul’tharok Trash

In this wing, you’ll navigate platforms connected by shadowy gates. Sometimes you must use an orb dropped by Portal Guardians to choose the path—always choose the right side by placing the orb accordingly.

Shackled Soul
Bind Soul channels on random players—interrupt or crowd control this.

Portal Guardian
Immune to CC. Soulstorm is a heavy party damage channel—use healing and defensive cooldowns. Shadow Vulnerability debuffs random players—use curse dispels and defensives during Soulstorm. These mobs drop orbs to select your gate path.

Bone Magus
Starts with Bone Shield absorbs—use purges to remove. Interrupt Bone Spear casts.

Maniacal Soulbinder
Necrotic Bolt Volley hits all players—keep it interrupted. Interrupt random Necrotic Bolt casts.

Soulforged Bonereaver
Immune to CC. Avoid Bone Spikes’ circles and stay clear during Bonestorm whirlwind.

Nefarious Darkspeaker
Immune to CC. Avoid Death Winds line attack. Curse of Desolation targets random players—use curse dispels or avoid circles. Interrupt Spirit Frost targeting the tank.


Kul’tharok Boss Fight

The boss stays in the room’s center. Avoid standing too far inside his hitbox—there’s a damaging puddle beneath him.

Stay loosely spread as two players will get Well of Darkness DoT, which splashes to anyone within 4 yards. Those affected should use defensives to mitigate damage.

Death Spiral spawns orbs rotating around the arena that damage players on contact. Ranged players should stand near the platform edges where orbs move slower.

At full energy, Draw Soul deals ticking damage for 8 seconds then spawns Lost Souls at each player’s location. If adds reach the boss, they explode dealing heavy damage. When this happens, group up near the platform edge, crowd control the adds, and cleave them down.

Keep interrupting random Necrotic Bolt casts to reduce damage. Tanks should use defensives and face the Necrotic Eruption frontal away from the group.

Gorechop Trash

Blighted Sludge-Spewer
Interrupt every Withering Discharge cast; use Disease dispels if missed. Use spare interrupts on Decaying Filth or dispel its debuff. Stay loosely spread to avoid cleaving from Leaping Thrash.

Diseased Horror
Interrupt or crowd control Meat Shield channel. Decaying Strike targets the tank—use defensives and dispel Disease if possible.

Putrid Butcher
Devour Flesh targets the tank—using a defensive reduces healing on the mob.

Disgusting Refuse
Stay spread out to avoid cleave from Leaping Thrash. Avoid Disgusting Burst circles created on death; dispel Disease if hit.

Rancid Gasbag
Immune to CC. Healers should prepare for constant AoE pulses. Avoid Vile Eruption frontal attacks and puddles of Rancid Bile under random players; dispel Disease as needed.


Gorechop Boss Fight

The boss uses Meat Hooks sweeping the arena in two waves with safe gaps. Hooks apply Jagged Gash and drag players hit across the room—dodge carefully through safe zones.

Each hook wave summons two Oozing Leftovers. These mobs lose health over time and use Leaping Thrash, so spread out to avoid cleave. They leave Coagulating Ooze puddles on death, so try to kill them together to conserve space and make dodging hooks easier.

Tenderizing Smash grips all players, then deals lethal AoE—use mobility spells to escape in time.

Tanks should prepare defensives for Hateful Strike, a heavy physical hit.

Mordretha, the Endless Empress Boss Fight

After clearing the interior bosses, return to the first boss room to face Mordretha. Combatants line the arena edges, knocking back players who get too close—avoid them to prevent fatal pushes.

Key Avoidances:

  • Dark Devastation: A random target frontal attack that rotates during the channel—give it plenty of space.

  • Grasping Rift: Spawns randomly and pulls players toward it for 6 seconds. Anyone near gets Death’s Grasp Curse—use movement abilities to avoid this.

Tank Notes:

  • Reaping Scythe is a dangerous tank buster mixing shadow and physical damage. Keep your defensive cooldowns timed well for every cast.

Group Damage:

  • Manifest Death applies a 6-second DoT to all players, splashing to those within 6 yards. When it explodes, it spawns a Deathwalker add beneath each player. These adds cast Death Bolt at random targets and must be interrupted.

  • Spread out loosely to avoid cleaving your group. After adds spawn, quickly interrupt and gather them under the boss, then use AoE crowd control to stop casts while cleaving them down.


At 50% health, Mordretha casts Echoes of Carnage, delivering a large shadow hit followed by small pulses every 2 seconds for the rest of the fight. This is the fight’s biggest hit—use defensives here.

This also causes Ghostly Combatants to spawn throughout the arena. They use Echo of Battle (8-yard AoEs) and Ghostly Charge (line attacks from four mounted combatants). Stay alert and avoid these attacks.

Summary and Final Tips

This dungeon requires careful coordination and awareness, especially during multi-boss fights where damage must be balanced to avoid stacking dangerous debuffs. Always be mindful of positioning, interrupt rotations, and dispel priorities, as these greatly reduce group damage.

Use defensive cooldowns strategically, especially during big hits from bosses like Sathel’s Searing Death, Dessia’s Mighty Smash, Xav’s Brutal Combo, Gorechop’s Meat Hooks, and Mordretha’s Echoes of Carnage.

Maintain loose spreading during fights that involve splash damage or cleave to minimize risk, and stack only when necessary, such as during the Draw Soul phase against Kul’tharok or when gathering adds in the final boss fight.

Mobility is key in several encounters to avoid lethal attacks and environmental hazards—make sure players have movement abilities ready.

Finally, practice the PUG-friendly route and use addons to optimize pulls and crowd control for smoother runs.
